	Tell the receptionist whether this is a routine visit or if you wish to have a comprehensive or Physical Exam.
	Tell the receptionist the problem for which you wished to be seen.
	Notify the receptionist if your insurance has changed
	Urgent appointments may be scheduled with one of our Physician Assistants in order to accommodate your needs.
	All laboratory work needs to be scheduled so that we may have your chart at that time.

What to bring with you for your appointment

	Your current Medication list
	A list of questions you may wish to ask your provider
	If you are a new patient download our new patient forms so that you may have these completed prior to your visit.
	Your Insurance card
	Your Lab orders if you are having lab work done
	Payment for your copay or deductible.
